To receive a Gold Individual or Gold Family Member discount,
Gold Members must have a household member on the teams active roster.
Team Practices are scheduled based upon availability.
Gold Members may schedule team practices 15 days out.
Non-members may schedule team practices 10 days out
All players on each team MUST sign a waiver prior to participating.
Teams must fill out the following form below with names of each player participating with Team Rentals. If more than 10 players are on a team, please submit an additional electronic form for the remainder of the players.

All Parents/Guardians of each player MUST complete a WAIVER for each play on the Team.
